You may request your Boulder accident report in person at the Boulder Police Department, located at 1805 33rd Street, Boulder, Co 80301. Boulder accident reports cost $5. Currently, the Records Department is open from 7:00 AM to 7:00 PM Monday through Friday.

Obtaining a Boulder Accident Report Online

Boulder accident reports are available online at the City of Boulder Website. The Boulder Police Department also provides a form to complete, which you can email to pdrecords@bouldercolorado.gov.

Obtaining a Boulder Accident Report Through the Mail

You may request a copy of your Boulder Accident Report through the mail. You will need to request a Boulder Car Accident Report request form. You’ll then fill it out and mail it back to the Boulder Police Department Records Department Unit, 1805 33rd Street, Boulder Colorado, 80301. The city of Boulder asks that you enclose a stamped, self-addressed return envelope and a check. The check should be for $5 made out to the City of Boulder for the first five pages of the report. Include an additional $0.25 for every page after that.
